Lane Health is committed to ensuring that no fees are incurred by our members, and we will be covering the HSA transfer fee. If you were charged this fee, please submit a reimbursement request to receive the amount refunded.

How to Request Reimbursement

To request reimbursement, we'll need the following information from you:

  1. First & Last Name
  2. Date of Birth
  3. Email Address
  4. Employer Name

Next Steps: Once you've successfully submitted the request form we expect the reimbursement process to be available late March. Please keep an eye out for further updates via email.
